Keeping You Safe: One LDRP’s Journey
A 38 bed LDRP, 113 FTEs, a full staff of physicians and midwives delivering over 4,200 babies a year and now the scene is set for a perfect storm. Things go wrong in every medical setting; when things go wrong in the OB setting, the consequences can be devastating. Members of our team are collectively invested in seeing that our environment is one where the focus is that of patient and caregiver safety. This can be a daunting task. Providing a “just culture” with education necessary to keep the bedside clinician informed is a challenge our organization has achieved. From documentation to disclosure, change occurs daily as we advocate for best practices. This presentation will highlight key points along with outcome measurement and strategies we have found successful.
Highlights will include the education and implementation of the following;
·        SRS (Safety Reporting System)·        Team Training
·        Clinical Decision Support System
·        CD Training Development
·        Simulations: shoulder dystocia, post partum hemorrhage, infant abduction
·        IHI Initiatives: induction and vacuum bundles
·        Staff  safety survey
We continue to follow and capture data that provides immediate feedback through our CDSS. Process capture will be shared along with specific outcomes.
The National Patient Safety Foundation includes just culture as it describes the basic principles of a patient safety culture, highlighting the culture of learning, justice, and accountability. [NPSF Congress – May 2007]. As consumers advocate for their care or those of their loved ones, leadership must empower the bedside clinician with the necessary systems and processes to accomplish this charge – one that is welcomed by both the patient and the provider.
