Nurses in the Mother/Baby unit at Kettering Medical Center (KMC), Kettering, Ohio, have been using electronic documentation for over 13 years. However, documentation of patient discharge education has continued on a triplicate paper copy checklist. The unit's Performance Improvement (PI) Committee recognized the need to transition patient education to be electronically recorded and instantly transferred to an electronic discharge summary, awaiting printing for patient discharge. Second, the unit's PI committee addressed standardization of Mother/Baby patient education across not only Mother/Baby at KMC, but also the Mother/Baby 'sister unit' in the hospital system's network. Third, PI committee members dedicated their improvement activities to reflect evidence-based approaches supported by professional guidelines of AWHONN, ACOG, and AAP that, in turn, would translate science to support excellence in outcomes and nursing practice. To date, an electronic template has been developed by a combined task force of staff nurses, nurse educators, and nursing informatics specialists that documents patient education specific to mode of delivery, feeding preferences, infant gender, and maternal/infant diagnoses. The template provides a discharge patient summary, instruction record, and recommendations of the patient's health care provider. Modes of education delivery were developed including but not limited to classroom format, televised instruction, individualized instruction and demonstration, and written instructional materials. Patient education processes and documentation methods were individualized for the patient, yet standardized across the network hospital system. Patient and Registered Nurse satisfaction were evaluated.
