Title: Welcome Aboard and Homeward Bound - The NICU Family's Journey for a Safe Voyage to Discharge

Submission Description:- Identify three creative tools to educate parents about their babies' special needs in the hospital and at home.
- Identify engaging ways to deliver education to staff and high risk families.
- Discuss techniques to increase staff and parent satisfaction with the discharge process, parent involvement, communication with the entire team, and documentation.
Purpose for the program: Improve parent and staff satisfaction with the discharge process and complex follow up care for the very premature or sick infant. Involve parents in the plan of care and daily care of their babies, and their personal preferences early on.
Proposed change: Standardize teaching, timing of education, documentation and communication of education in order to better prepare parents for discharge.
Implementation, outcomes and evaluation: Implementation using PDSA cycles, staff and patient satisfaction as well as chart ausits were used for measurement and feedback.
Implications for nursing practice: best practice was identified. We continue to maintain the gains by monitoring satisfaction levels and through random chart audits.
