Title: H2 - Standardization Of The Second Stage Of Labor: A Perinatal Quality Perspective

Disciplines: Advanced Practice (AP), Childbearing (CB), Professional Issues (PI), Women’s Health (WH)

Learning Objectives:
  1. Describe specific strategies for implementation of a standardized second stage of labor guideline at various hospital types – academic, urban and rural.
  2. Describe the implementation process from a quality improvement perspective including: establishing current state of clinical practice, education of staff and providers, data collection and analysis as well as obstacles and lessons learned.
  3. Identify maternal and newborn outcomes resulting from the use of an evidence-based guideline to standardize the management of the second stage of labor including improved total cost of care.
Submission Description:
In women with epidurals, delaying pushing, until the urge to push is present is an effective strategy to improve maternal and neonatal outcomes. The experience of a community hospital following implementation of a standardized second stage evidence-based guideline showed significant improvements. The guideline was subsequently implemented at an academic medical center and results compared. This presentation will discuss the implementation process from a quality perspective, sharing the results on different patient populations and provider mix.
