  • 3.Integrate trigger tool and hemorrhage identification magnet into initial assessment, delivery and post-partum care.

  • Submission Description:
    The early recognition and identification of patients with risk factors for Post-Partum Hemorrhage may decrease the number of patients who actually experience a hemorrhage. Collaboration between providers and nurses focus on identification of the patients at risk upon admission and visual cues alerting individuals to patients at risk throughout the hospital stay.

    The Association of Women's Health, Obstetric and Neonatal Nurses is accredited as a provider of continuing nursing education by the American Nurses Credentialing Center's Commission on Accreditation.
