  • 1.Implement trigger tool to identity patients at medium and high risk for post-partum hemorrhage.
  • 2.Transform the quality of care and responsiveness of providers through collaboration of providers and nurses on trigger tool.
  • 3.Integrate trigger tool and hemorrhage identification magnet into initial assessment, delivery and post-partum care.

    • Submission Description:
    The early recognition and identification of patients with risk factors for Post-Partum Hemorrhage may decrease the number of patients who actually experience a hemorrhage. Collaboration between providers and nurses focus on identification of the patients at risk upon admission and visual cues alerting individuals to patients at risk throughout the hospital stay.
