Discipline: Childbearing (CB), Newborn Care (N)

Learning Objectives:
  • 1.Identify barriers to access of patient information across two healthcare organizations.
  • 2.List key partnerships needed to enhance and improve collaboration between Obstetrical and Pediatric providers to improve quality of care and outcomes
  • 3.Discuss challenges and opportunities for successful communication and access to important patient care information.

  • Submission Description:
An obstetrical service with 3600 births annually was challenged with coordinating and providing access to patient care information to over 100 Pediatric and Obstetric providers in a hybrid computer system. This presentation will present an innovative program that has successfully surmounted the challenges of identifying high risk newborns, facilitating access to patient care information and spanning the gaps created by two different healthcare organizations caring for the obstetrical patient and her fetus.

The Association of Women's Health, Obstetric and Neonatal Nurses is accredited as a provider of continuing nursing education by the American Nurses Credentialing Center's Commission on Accreditation.
