Title: Perinatal Case Management-Changing the Outcome!

Learning Objectives:
  • Participants will define Perinatal Case Management and identify their roles and responsibilities.
  • Participants will identify the goal of a Perinatal Case Management program.
  • Participants will identify the three leading causes of infant mortality.

  • Submission Description:
    Our Perinatal Case Management Program began in April of 2012; the goal of this program is to decrease infant mortality and improve perinatal outcomes. Since the inception of this program our nurses have case managed over 2000 high risk pregnant women and we have experienced remarkable outcomes. We are proud to announce that since September 2014 we have had only 14 babies born preterm in one of our targeted high risk zip codes and no infant fatalities.
